Enjoying unique lifestyle opportunities, St Helena Island is a self-governing overseas territory of the United Kingdom. Although better placed to develop economically and socially as a result of opening our international airport in 2017, we are not at the stage of self-sufficiency and therefore remain dependent on UK government aid. The 22-bed General Hospital in the capital, Jamestown, covers all areas of acute medical and surgical care.

The island has a very high prevalence of type 2 diabetes, with 30% of the population on hypoglycaemic medication. Many of these people are also overweight or obese. Glycaemic control is often poor, resulting in common presentations with complications of diabetes. This post is part of a new initiative to try to improve the management of diabetes on St Helena, providing more dedicated diabetes care, and empowering people to manage their own condition.

The DSN will be instrumental in the promotion of lifestyle changes through, diet, exercise and weight management. They will provide intermediate services, and support primary care services. This will involve  collaboration with GP services, hospital management, community nursing, schools, social care, secondary care colleagues, and other agencies. As an expert resource for health care professionals, family and carers the DSN will develop and participate in local public health diabetes initiatives.

The post-holder will be expected to manage a complex caseload in a primary care setting, run diabetes clinics, and formulate individualised patient treatment care programmes in conjunction with the patient and multidisciplinary team.  They will be responsible for assessing patients’ diabetes needs, and  referring to secondary care, multidisciplinary teams and voluntary services where appropriate. They will request and interpret diagnostic investigations and inform patients and GPs of results, acting as the patient’s advocate when relevant

The DSN will deliver education programmes and act as a clinical expert, offering advice and support to community based staff and junior members of the team. You must identify training needs as you initiate and participate in up-to-date training programmes for a variety of multidisciplinary professionals, including general practice staff, community nurses, residential and nursing homes.

A registered nurse in good standing with a national registering body, the successful candidate must have good diabetes knowledge supported by specialist training in diabetes management.

In addition, the successful candidate must demonstrate our core values of fairness, integrity and teamwork and use their diverse expertise and experiences to ensure that their time on the Island has the broadest benefit to St Helena.  As an officer funded through the Technical Co-Operation Programme they will be expected to share their knowledge and skills beyond the workplace contributing to workforce, community and capacity development in St Helena.
